Stroke:白质高信号–调整的临界梗死阈值预测患者预后

这项概念验证研究的结果表明,白质高信号病变程度影响预测梗死阈值。使用白质高信号调整的梗死阈值可以从有较大的脑梗死面积的患者中挑选出预后较好的个体。

最近,用卒中病灶体积阈值预测卒中预后的方法日渐广泛,然而,对于梗死体积临界阈值与预后之间关系的影响因素缺乏主要的研究数据。近日,卒中领域权威杂志《Stroke》上发表了来自美国麻州大学医学院神经内科的Jatinder Patti及其团队的研究文章,试图确定损伤阈值的最佳预测结果是否取决于先前的白质高信号病变程度。

研究者纳入了从2014年1月至12月期间因前循环缺血性卒中而住院的414例患者,对其脑梗死体积通过磁共振成像进行定量分析,白质高信号病变体积按法泽卡斯量表进行分级,并且分为无至轻度组与中度至重度组两组。计算ROC曲线以确定梗死体积阈值来完美预测90天后的临床预后,对相关的混杂因素进行调整后,多因素logistic回归分析被用来确定临界损伤阈值是否能够独立地预测90天后的最佳预后。


在整个研究队列中,预测90天后预后的脑梗死体积阈值被为(标准阈值)≤29.5ml(改良Rankin量表为0–1)、≤29.9ml(改良Rankin量表为0–2)和≤34.1ml(改良Rankin量表为0–3)三个等级。对于无至轻度白质高信号病变程度的患者,白质高信号调整的临界梗死阈值明显大于标准的梗死阈值,在充分调整的多变量回归模型中,白质高信号调整的梗死阈值能够跟标准阈值一样正确地预测患者预后。
