Crit Care:单核细胞分布宽度作为急诊科脓毒症早期诊断的指标

MDW与WBC相结合使败血症的诊断更准确。研究人员建议使用MDW作为系统筛查指标,与qSOFA评分一起使用,以提高ED对脓毒症诊断的准确性。

脓毒症早期诊断已成为急诊(ED)的主要挑战之一。脓毒症生物标志物的测量在目前的临床实践中主要以提高诊断准确性为主。单核细胞分布宽度(MDW)是最近一种新的脓毒症生物标志物。

近日,危重病医学领域权威杂志Critical Care上发表了一篇研究文章,研究人员旨在评估MDW在ED检测脓毒症方面的性能,并与降钙素原(PCT)和C反应蛋白(CRP)的效能进行比较。


研究人员纳入了法国和西班牙的2个ED中接受全血细胞计数在内的初级评估的受试者,并根据Sepsis-2和Sepsis-3标准进行分类。


MDW、WBC和MDW+WBC在脓毒症诊断方面的性能 


该研究总共分析了1517名患者:837名男性和680名女性,平均年龄为61±19岁,260名(17.1%)患者被归类为Sepsis-2,144名(9.5%)患者被归类为Sepsis-3。采用MDW和MDW与WBC结合来诊断Sepsis-2的AUCs[95%置信区间]分别为0.81[0.78-0.84]和0.86[0.84-0.88]。对于Sepsis-3,MDW性能为0.82[0.79-0.85]。MDW联合WBC在脓毒症预检概率低的患者亚组中对Sepsis-2的效能为0.90[0.84-0.95]。使用MDW联合WBC检测脓毒症的AUC与单独的CRP相似(0.85[0.83-0.87]),并超过PCT。结合生物标志物并没有改善AUC。与正常MDW相比,异常MDW使Sepsis-2的几率增加5.5[4.2-7.1,95%CI]和Sepsis-3的几率增加7.6[5.1-11.3,95%CI]。
 
由此可见,MDW与WBC相结合使败血症的诊断更准确。研究人员建议使用MDW作为系统筛查指标,与qSOFA评分一起使用,以提高ED对脓毒症诊断的准确性。
