Lancet oncol:循环肿瘤DNA分析足以指导乳腺癌靶向治疗!

循环肿瘤DNA(ctDNA)检测或许可为晚期癌症的基因组谱提供实时评估,且不需要重复进行肿瘤活检。本研究旨在评估ctDNA检测评估晚期乳腺癌的精确性,以及ctDNA检测筛选适合突变靶向治疗的患者的能力。

本研究是一项多中心、开放性的2a期试验,招募了年满18岁的组织学确诊的晚期乳腺癌患者。要求患者既往至少接受过一次针对晚期乳腺癌的治疗或在新辅助/辅助化疗后12个月内复发。将患者分到4个与ctDNA中的突变相匹配的平行治疗组:A组,ESR1突变(采用氟维司群 500 mg,肌注);B组,HER2突变(Neratinib 240 mg,口服,如果雌激素受体阳性,加用标准剂量的氟维司群);C组,AKT1突变和雌激素受体阳性(capivasertib 400 mg,口服,联合标准剂量的氟维司群);D组,AKT1突变和雌激素受体阴性或PTEN突变(capivasertib 480 mg,口服)。主要终点是客观缓解率。

(B组患者HER2基因的突变情况)

2016年12月21日-2019年4月26日,筛选了1051位患者,其中1034位有ctDNA数据。ctDNA数字PCR和靶向测序的一致性高达96%-99%。数字PCR ctDNA测序检测突变的总灵敏度为93%,对同期活检组织的灵敏度为98%。在所有队列中,中位随访14.4个月。B组和C组达到或超过了目标缓解数,缓解率分别为25%(5/20)和22%(4/18)。A组和D组未达到目标缓解例数,缓解率分别为8%(6/74)和11%(2/19)。最常见的3-4级不良反应为γ-谷氨酰转移酶(16%,A组)、腹泻(25%,B组)、疲劳(22%,C组)和红疹(26%,D组)。17例重度不良反应发生于11位患者,1例由4级呼吸困难导致的治疗相关死亡(C组)。

ctDNA检测可提供精准、快速的基因型鉴定,能够帮助乳腺癌患者选择突变靶向疗法,具有足够的临床有效性,可应用于日常临床实践。靶向HER2和AKT1罕见突变显示出临床相关活性,提示这些突变或可成为乳腺癌治疗的靶点。
