Rheumatology:在急性关节炎患者中,靶向超声扫描方案对痛风和CPPD的诊断具有很高的准确性

确定一种超声(US)扫描方案,用于诊断不明原因的急性单/少关节炎患者的痛风和CPPD,以提高诊断的准确性。

目的:确定一种超声(US)扫描方案,用于诊断不明原因的急性单/少关节炎患者的痛风焦磷酸盐关节病(CPPD),以提高诊断的准确性。

方法:连续入选急性单/少关节炎患者,要求在最临床受累的关节(目标关节)进行关节抽吸。每个患者在关节穿刺前均进行了超声检查。计算了诊断痛风和CPPD的不同超声结果和扫描方案的准确性。

结果:该研究共纳入161名受试者(32名痛风患者、30名CPPD患者和99名疾病对照)。

US结果对痛风(0.92-0.96)和CPPD(0.90-0.97)具有高度特异性,对痛风(双轮廓征和痛风石)的敏感度范围是0.73-0.85,CPPD(玻璃样软骨和纤维软骨沉积)的敏感度为0.60-0.90。

双侧两个关节(痛风:膝盖、MTP1js;CPPD:膝盖、手腕)+目标关节的超声评估具有极好的诊断敏感性(痛风:0.91,CPPD:0.93)和特异性(痛风:0.91、CPPD:1.89)。

与目标关节的超声评估相比,该靶向超声扫描方案产生了更高的诊断准确性(痛风,AUC:0.91 vs AUC:1.84,p = 0.03;CPPD,AUC:0.93,与AUC:1.84相比,p = 0.04),除非目标关节是痛风患者的膝关节或MTP1j和CPPD患者的膝或腕关节。

结论:双侧两个关节+目标关节(除非是膝关节、MTP1j或腕关节)的靶向超声扫描方案在诊断急性单/少关节炎患者的结晶性关节炎方面显示出极好的准确性(>90%)。
