骨关节炎(OA)是最常见的关节炎形式,也是老年人慢性关节疼痛和残疾的主要愿意。它是一种全器官疾病,可导致包括骨、软骨、半月板、滑膜、韧带和脂肪垫等多种关节组织的异常。膝关节内有4个囊内鞘外的脂肪垫,即髌下脂肪垫(IPFP)、股四头肌脂肪垫(QFP)、股前脂肪垫(PFP)和膝周脂肪垫(PCFP)。作为唯一的后方脂肪垫,PCFP填补了前交叉韧带(ACL)和后交叉韧带(PCL)之间的空隙,并且在后方和下方被滑膜覆盖。在矢状面中,它呈现为三角形。3个前部脂肪垫特别是IPFP,在OA中的重要性已被多项研究所证实。相比之下,很少有研究关注PCFP的信号强度改变。到目前为止,PCFP对OA的影响还尚未被阐明

近日,发表在European Radiology杂志的一项研究探讨了PCFP信号强度改变和最大面积与4年内影像学骨关节炎(ROA)发病率之间的关系OA的病理生理阐明及治疗提供参考依据

所纳入的病例膝关节(n = 355)由12至48个月随访期间发生的ROA定义,并与对照膝关节(n = 355)进行性别、年龄和影像学状态的匹配。MRI被用来评估PCFP信号强度的改变和PCFP在P0(ROA发病时间)、P-1(P0前1年)和基线的最大面积。应用条件逻辑回归分析来评估PCFP指标与ROA发病风险之间的关联。 

参与者的平均年龄为60.1岁,66.9%为女性。在多变量分析中,在三个时间点测量的PCFP信号强度改变(OR[95%CI]:分别为1.28[1.10-1.50]、1.52[1.30-1.78]、1.50[1.27-1.76])和PCFP最大面积(OR[95%CI]:分别为1.21[1.03-1.42]、1.27[1.07-1.52]、1.37 [1.15-1.62] )与发生ROA显著相关。

本研究表明PCFP信号强度的改变和大小与4年内发生ROA的风险增加显著相关,表明在ROA的发生发展中发挥着一定的作用。
