最近来自南方医院肝病中心的侯金林教授领衔的全球大规模临床研究在顶级期刊《Journal of Hepatology》上发表,该研究发现了新的肝癌风险预测模型——aMAP评分,这是全球首个跨病因(乙肝、丙肝、非病毒性肝病等)、跨种族(亚洲人、西方人等)的针对慢性肝病患者的肝癌风险预测评分模型,基于患者年龄、性别这两个基本信息和白蛋白、胆红素和血小板这三个实验室常见检测指标,能够准确、可靠、简便地预测不同肝病人群HCC发生风险。

研究方法

该研究纳入全球11个前瞻性长期随访队列的17,374名的慢性肝病患者,包括13,088例经治慢乙肝(CHB)患者(10578例为亚洲人;2510例为白种人),3566例经治丙肝(HCV)感染患者和720例非病毒性肝病(NVH)患者。将患者分为1个训练队列(3688例亚洲CHB患者)和9个包括不同病因和种族的验证队列(包括3个CHB亚洲人队列、2个CHB白种人队列、3个HCV队列和1个NVH队列,共13686人)。

使用SPSS(20.0)、R软件进行分析,使用多因素Cox回归分析筛选HCC相关危险因素并构建预测模型;采用一致性检验(C-index)评估模型的效能。

研究结果

aMAP评分模型指标:年龄、性别、白蛋白-胆红素评分(ALBI)和血小板指标

训练队列中,中位随访42.7个月(IQR:35.5,54.4)内共95例患者发生了HCC。1,3,和5年的累积HCC发生率分别为0.4%,1.8%和3.7%。单因素分析发现年龄,性别,肝硬化,HBeAg状态,定量HBsAg水平,肝硬度测量(LSM),ALBI和血小板与5年随访的HCC的发生相关。

由于临床上对肝硬化的诊断相对主观,大多数基层医疗机构无法进行肝硬度值的检测,且病毒学指标在抗病毒治疗过程中的变化较大,因此该研究将纳入HCC风险预测模型的指标限定在年龄、性别、ALBI和血小板这些非病毒学指标。利用这4个变量在多因素Cox回归模型中的系数加权,构建了age-Male-ALBI-Platelets (aMAP) 评分模型。

评分范围标准化为1-100, aMAP 评分的计算规则为:((0.06 × 年龄 + 0.89 × 性别 (男性:1, 女性:0) + 0.48 × ((log10 总胆红素 × 0.66) + (白蛋白× -0.085)) -0.01 × 血小板) +7.4) /14.77 × 100, 其中年龄以年为单位,其它检测指标的单位分别为总胆红素(μmol/L)、白蛋白(g/L)和血小板(103/mm3)。

aMAP模型对不同类型和种族的慢性肝炎患者有良好的预测HCC能力

该评分模型在不同病因的肝炎患者和不同种族的肝炎患者中评估HCC风险均表现出色(C-index:0.82-0.87)。

按照最佳临界值50和60,将aMAP评分的3662例训练队列患者分为低(0-50)、中(50-60)、高风险组(60-100),分别为2158例(58.9%)、1181例(32.3%)和323例(8.8%),其5年累积HCC发生率分别为0.8%、4.2%和19.9%(P <0.0001)。临界值为50时,灵敏度为86.5%,阴性预测值(NPV)为99.5%。临界值为60时,特异性为92.2%,阳性预测值(PPV)为13.3%。

具有aMAP评分的13324例验证队列患者中,5255例(39.4%)、5348例(40.1%)和2721例(20.4%)患者被分为低、中、高风险组,其3或5年累积HCC发生率分别为0 - 0.8%、1.5 - 4.8%和8.1 - 17.8%。临界值50时,敏感度为85.7 -100%和阴性预测值为99.3-100%。临界值为60时,特异性为56.6-95.8%,阳性预测值为6.6-15.7%。

 

aMAP评分模型优于现有HBV相关HCC风险预测模型

在慢乙肝相关训练队列中,aMAP评分的C-index显著高于其他现有HBV相关HCC风险评分模型(vs. REACH-B, CU-HCC, LSM-HCC, mREACH-B, PAGE-B, mPAGE-B, P值均<0.05)。且在各种不同队列中,aMAP评分模型的预测能力也优于其他现有预测模型。

aMAP模型是全球首个跨病种、跨种族的慢性肝病HCC风险评分模型。不含病毒因素,且不受抗病毒治疗的影响,其评分仅纳入五个非常常见且容易获得的指标,简单,方便,可靠,因此可以在乙肝、丙肝、非病毒性肝病等各类人群中进行广泛的应用

aMAP评分中低风险组占总人群约45%,其每年HCC发生风险小于0.2%,提示近一半的慢性肝病患者不需要接受较为频繁的HCC检测,可显著减轻患者负担,并优化资源配置,提高高风险人群的监测频率。最终可实现患者个体化的肝癌风险监测,从而提高肝癌的早诊早治,最终减低死亡率,才能实现将死亡率降低65%的宏伟目标。
