Thorax:COPD患者吸入氟替卡松可增加其患严重肺炎风险

为了加深对这些问题的认识,来自加拿大蒙特利尔市麦克吉尔大学附属犹太总医院临床流行病学中心的Samy Suissa及其同事进行了一项研究,研究结果发表于2013年11月出版的《Thorax》杂志上。研究结果显示:COPD患者使用ICS会增加其患严重肺炎的风险。吸入氟替卡松可增加这种风险,且该风险的增加与其剂量相关。

研究者利用加拿大魁北克省健康保险数据库建立了一个新的患者队列。所有入选者均为在1990~2005年期间,新开始使用ICS治疗其慢性阻塞性肺疾病的患者。入选者被随访到2007年年底、或出现了一次严重的肺炎事件为止。这里研究者将严重的肺炎事件定义为:一个第一次需要住院治疗、或引起患者死亡的肺炎。

研究者使用巢式病例–对照分析法评估了受试者严重肺炎与其当前ICS使用相关的率比(RR);并在分析过程中对受试者的年龄、性别、呼吸系统疾病严重程度、以及合并症等因素进行了统计学校正。

该研究的主要结果为:该患者队列共包括了163 514例患者,其中20 344例在5.4年的随访期内出现了严重的肺炎事件,其年化发病率为2.4%。当前使用ICS与受试者严重肺炎患病率69%的增加相关,其RR为1.69。该风险随着ICS的长期使用而持续存在;但在停止使用ICS后,这种风险会逐渐下降,并于停用6个月后消失,后者的RR为1.08。严重肺炎的发生率在吸入氟替卡松的受试者中较高(其RR为2.01),且随其日剂量的增加而增加;但吸入布地奈德的患者出现严重肺炎的比例则明显较低,其RR仅为1.17。

该研究结果显示:COPD患者使用ICS会增加其患严重肺炎的风险。吸入氟替卡松可增加这种风险,且该风险与其剂量相关。尽管残余的混杂因素不能排除,但该研究结果与那些新近的随机试验结果具有一致性。
