对因新冠肺炎入院的糖尿病患者进行心脏脂肪组织指数和IL-6测定,可以帮助医生更好地识别具有高短期内病情迅速恶化和早期死亡风险的患者
无论是否有肥胖,糖尿病成人患者感染新冠病毒后发展成重症的风险都会增加。在2型糖尿病患者中,脂肪分布的特点是内脏和异位脂肪组织扩张,导致全身炎症,这可能参与推动新冠肺炎(COVID-19)时细胞因子风暴的发生。
该研究目的是明确心脏脂肪组织结合白细胞介素-6(IL-6)水平是否可以预测 COVID-19 糖尿病患者在入院后 21 天内的短期不良结果、死亡和 ICU 需求。
纳入了因COVID-19住院的81位2型糖尿病患者。在入院时检测了IL-6水平,并进行了具有总心脏脂肪组织指数 (CATi) 测量值的胸部CT。主要终点是入院21天内的死亡,次要终点是ICU需求。
根据短期预后分组的患者的CATi和IL-6水平
受试患者的中位年龄为66岁,73%的男性,中位体重指数(BMI)为27 kg/m2,20位患者死于COVID-19,20位患者需入住ICU,41位患者在入院第21天时接受常规护理。CATi和IL-6水平升高均与早期死亡率(OR分别是6.15[p=0.002]和18.2[p<0.0001])和ICU需求(3.27[p=0.01]和4.86[p=0.002])增加显著相关。而且,这些相关性不受年龄、性别、BMI以及肌钙蛋白-T水平和CT检查时的肺部病变扩张的影响。
根据CATi和IL-6预测患者短期死亡风险和ICU需求的表现
研究人员将CATi和IL-6水平联合作为乘性互动评分(CATi*IL-6)。将该评分的分界值定为≥6386分时,敏感性为0.90,特异性为0.87(AUC=0.88),早期死亡的OR为59.6(p<0.0001)。
总之,对因新冠肺炎入院的糖尿病患者进行心脏脂肪组织指数和IL-6测定,可以帮助医生更好地识别具有高短期内病情迅速恶化和早期死亡风险的患者。因新冠肺炎入院的具有高CATi的糖尿病患者,尤其是IL-6水平也高的患者,可能需要迅速启动抗炎治疗。
