AJG: 膳食维生素E的摄入与降低患消化系统疾病和非酒精性脂肪肝的风险有关

非酒精性脂肪性肝病(NAFLD)分原发性和继发性两大类,前者与胰岛素抵抗和遗传易感性有关,而后者则由某些特殊原因所致。
非酒精性脂肪肝(NAFLD)的特点是脂滴在肝细胞中堆积,影响了大约20-30%的普通人群。非酒精性脂肪肝是一种多系统疾病,是导致2型糖尿病(T2D)、心血管疾病、胰岛素抵抗和肾脏疾病的风险因素。目前还没有经批准的药物制剂可用于直接治疗非酒精性脂肪肝。对非酒精性脂肪肝最有效的治疗干预措施是改变生活方式,包括改变饮食习惯和饮食结构。越来越多的证据支持维生素E对非酒精性脂肪肝发展的潜在益处。维生素E是一种脂溶性维生素,具有抗氧化、抗炎和抗凋亡的特性。为此本项研究旨在探究膳食维生素E的摄入与非酒精性脂肪肝之间的关系。
为了完成本项研究,研究人员评估了所纳入患者的饮食中维生素 E 的摄入量和疾病表型,并评估了 NAFLD 发展过程中的维生素E水平,并对年龄、性别、BMI、吸烟、饮酒/每日总能量进行校正。同时将使用维生素E补充剂的参与者排除在研究之外。
研究结果显示:来自超过 210000 名参与者的数据表明,增加膳食维生素 E 与降低几种胃肠道疾病的发病率和降低总体死亡率有关。维生素 E 摄入量增加的糖尿病和超重受试者的 NAFLD的发生概率也会减少。每增加一个单位的维生素E摄入量,非酒精性脂肪肝ICD-10诊断的风险就会降低12.5%,增加维生素E的摄入对非酒精性脂肪肝的保护作用,并且在超重患者中也有一定程度的保护作用。诊断的非酒精性脂肪肝的患者摄入的维生素E量的比其他患者低20%。
本项研究证实补充维生素E用于非酒精性脂肪肝的治疗已显示出对非糖尿病患者有益的结局。有趣的是,维生素E的保护作用在超重或肥胖者中最为明显,而且饮食中维生素E摄入量的增加与总体死亡率的降低有关。
