Age&Ageing:我国学者:注意了——中老年甲状腺功能变化居然与颈动脉硬化有关!

甲状腺激素对心肌、外周循环系统和交感神经系统有着重要影响。

甲状腺激素对心肌、外周循环系统和交感神经系统有着重要影响。主要改变包括:增加心率、心肌收缩力、收缩压和平均肺动脉压、心输出量、舒张期松弛功能以及心肌耗氧量。降低全身血管阻力和舒张压。

既往研究表明,甲状腺功能与心脏和血管之间存在关联。然而,目前仍不清楚甲状腺功能的纵向趋势是否有助于动脉粥样硬化的发展。为此,来自我国医学科学院放射医学研究所和北京协和医学院的专家进行了一项队列研究,以考察中年和老年健壮受试者的甲状腺功能纵向趋势与颈动脉粥样硬化(CA)的关系。结果发表在Age&Ageing杂志上。

这项队列研究的参与者是从中国天津的天津慢性低度系统性炎症和健康队列研究中招募的。采用化学发光免疫法测量游离三碘甲状腺原氨酸(FT3)、甲状腺素(FT4)和促甲状腺激素(TSH)。使用颈动脉超声检查评估CA。随访期间每年对甲状腺功能和超声检查进行评估。多变量的Cox比例危害回归模型被用来评估甲状腺功能和发生CA之间的关联。

共有3181名参与者被纳入队列研究中。在7762人年的随访中,944人患上了CA,CA的发病率为122/1000人年。FT3、FT4每增加1个单位,完全调整后的CA风险分别增加34%、和22%(HR=1.34,1.22-1.47;HR=1.22,1.19-1.26),而TSH则没有影响(HR=0.92,0.77-1.09)(P<0.0001,<0.0001和=0.32)。

此外,研究人员观察到FT3和FT4的平均水平与发生的CA之间有类似的明显关联。然而,基线甲状腺功能与发病的CA没有关系。

这些发现表明,中老年人甲状腺激素的平均水平偏高与发生CA的风险较高有关。
