Nature:骨髓增生性肿瘤是如何具有遗传性的?

骨髓增生性肿瘤(MPNs)是一系列以成熟髓细胞过度生成为特征的血癌。虽然MPNs产生于体细胞驱动突变,但以往的工作表明,该病有相当大的遗传成分。MPNs患者的一级亲属患MPNs的风险会增加5-7倍,其家族风险大于乳腺癌(相对风险(RR)=3.5)、前列腺癌(RR=2.46)和结直肠癌(RR=2.25)。

遗传研究已经确定了一组有限的与MPN易感性相关的位点。然而,遗传性MPN风险的许多关键方面仍未定性,包括致病基因、细胞类型和涉及的潜在生物机制。

最近,研究人员在Nature杂志发文,其通过进行大规模的全基因组关联研究(3,797例和1,152,977名对照),确定了17个MPN风险位点(P <5.0×10-8),其中7个位点以前没有被报道过。

研究人员发现,MPN风险与来自不同血统的几种造血性状之间有一个共同的遗传结构;在HSCs可访问的染色质内,MPN风险变异有一个富集;MPN风险的增加与白细胞和其他克隆造血状态中较长的端粒长度相关--这共同提示了MPN风险与HSCs的功能和自我更新相关。

研究人员还使用基因图谱来确定与MPN风险相关的HSC生物学调控因子,并通过靶向变异到功能测定表明,CHEK2和GFI1B在改变HSCs的功能以赋予疾病风险中发挥作用。

总的来说,这项研究的结果揭示了一个以前未被重视的机制,即通过调控HSC功能来实现遗传性MPN风险。
