NEJM:非奈利酮对2型糖尿病患者慢性肾脏病结果的影响

2型糖尿病是引起慢性肾脏病(CKD)的主要原因。国际指南中明确指出,对于患有CKD的2型糖尿病患者,应当控制高血压和高血糖,以及使用肾素-血管紧张素(RAS)阻滞剂(血管紧张素转换酶(ACE)抑制剂或血管紧张素受体阻滞剂(ARB)),最近更新的则是钠葡萄糖共转运蛋白2(SGLT2)抑制剂的应用。然而,即使采取了建议的治疗方法,CKD进展的风险仍然存在,更先进的治疗方法迫在眉睫。


有足够的证据表明,在通过炎症和纤维化导致心脏病和心血管功能障碍的心血管疾病(包括CKD和糖尿病)中,盐皮质激素受体过度活化起到主要的病理生理作用。尽管荟萃分析显示,CKD患者在接受类固醇眼皮直激素受体拮抗剂治疗后,尿蛋白或尿蛋白排泄率可降低31%,但目前仍缺乏硬性临床结果的数据。


非利奈酮是一种非类固醇的选择性盐皮质激素受体拮抗剂,它比类固醇盐皮质激素受体拮抗剂具有更强的抗炎和抗纤维化作用。研究显示,非利奈酮可以降低经RAS阻滞剂治疗的CKD患者的尿白蛋白/肌酐比值,同时对血浆钾水平有着比螺内酯更小的影响。减少肾功能衰竭和糖尿病性肾脏疾病疾病进展中的细肾上腺素试验(FIDELIO-DKD)旨在测试以下假说:细肾上腺素可减慢CKD进程并降低心血管疾病 晚期CKD和2型糖尿病患者的发病率和死亡率。非利奈酮在糖尿病肾病患者中减少肾脏衰竭和疾病进展(FIDELIO-DKD)试验旨在测试以下假说:在患有严重CKD和2型糖尿病的患者中,非利奈酮可减缓CKD的进展并降低心血管疾病的发病率和死亡率。

 


在这项双盲试验中,研究人员将5734名CKD和2型糖尿病患者以1:1的比例分为两组,分别接受芬利奈酮或安慰剂治疗。 患者需符合的条件为:尿白蛋白与肌酐之比(白蛋白以毫克为单位,肌酐以克为单位)小于或等于300,估计的肾小球滤过率(eGFR)为25-60毫升/分钟/1.73平方米体表面积和糖尿病性视网膜病变;或者尿蛋白/肌酐比为300至5000,eGFR为25-75毫升/分钟/1.73平方米体表面积。 所有患者均接受了RAS阻滞剂的治疗,在随机分配之前已将药物剂量调整至制造商标签上的最大剂量,且没有引起不可接受的副作用。 通过事件发生时间分析评估的主要复合终点为肾衰竭,eGFR持续下降至少40%(基线)或因肾病致死。在事件分析中评估的主要的次要综合结局是心血管原因、非致命性心肌梗塞、非致命性中风或因心力衰竭住院引起的死亡。


在2.6年的中位随访期间,非利奈酮组的2833例患者中有504例患者(17.8%)发生了主要预后事件,安慰剂组的2841患者中有600例(21.1%)发生了主要预后事件(危险比,0.82; 95%可信度) 区间[CI],0.73至0.93; P = 0.001)。关键的次要转归事件分别为367例(13.0%)和420例(14.8%)(危险比,0.86; 95%CI,0.75至0.99; P = 0.03)。总体而言,两组的不良事件发生频率相似。与非利奈酮相比,试验方案中与高钾血症相关的中止发生率高于安慰剂组(分别为2.3%和0.9%)。

 

本试验中,与安慰剂组相比,接受非利奈酮的CKD和2型糖尿病患者的初次结局事件(肾衰竭,eGFR与基线相比持续降低≥40%或因肾脏原因死亡)的风险较低。非利奈酮组的患者发生次要关键事件(因心血管原因死亡,非致命性心肌梗塞,非致命性中风或因心力衰竭住院)的风险也较低。 这些结果表明,在患有CKD和2型糖尿病的患者中,应用非利奈酮可能是肾脏和心血管保护的有效治疗方法。
