Radiology:CTA测算门静脉压力梯度来非侵入性诊断门静脉高压

本研究旨在建立并验证评价基于CT血管造影图像肝门静脉压力梯度(HVPG)计算机模型(虚拟HVPG)在非侵入性诊断肝硬化患者门静脉高压的价值。

本研究共纳入了102例受试者,不同病因所致肝硬化患者行经颈静脉HVPG测量、多普勒US和CT血管造影。利用3D重建模型和计算机流量动态算法建立虚拟HVPG。

结果为,在训练组(n=29),虚拟HVPG在预测临床显着门静脉高压(SCPG)的ROC曲线下面积为0.83 (95% CI: 0.58, 1.00)。该诊断能力在验证组得到验证,曲线下面积为0.89 (95% CI: 0.81, 0.96)。组间和组内阅片者一致性分别为0.88、0.96,表明虚拟HVPG指标具有良好可重复性。虚拟HVPG和侵入性HVPG间具有良好的相关性(R = 0.61, P < .001),以低于7.3 mm Hg排除CSPH,以13.0 mm Hg诊断CSPH。

本研究表明,虚拟肝门静脉压力梯度(HVPG)计算机模型表明其与侵入性HVPG具有良好相关性。虚拟HVPG同时对非侵入性诊断肝硬化患者临床显着门静脉高压具有较高价值。
