J Endod:使用MTA或氢氧化钙实施去龋露髓的直接盖髓术疗效评估:1年随访调查

这篇临床随机对照试验的目的是为了比较龋源性露髓牙齿及可复性牙髓炎使用MTA或氢氧化钙(CH)实施直接盖髓术(DPC)的成功率及术后疼痛。研究将64颗去龋后的恒牙随机分为2组:CH和MTA (n = 32 in each group)。使用标准治疗程序覆盖暴露牙髓。一级疗效为术后12个月随访的成功率,二级疗效为术后7天的疼痛情况。术后3, 6和12个月临床和放射线影像评估患牙。采用可视化模拟量表每隔24小时记录术后疼痛情况,直至术后7天。结果显示,12个月后随访共分析检查了56颗牙齿,29颗施以CH和27颗施以ProRoot MTA (Dentsply Tulsa Dental, Tulsa, OK)。CH的成功率为69%和ProRoot MTA的成功率为93% (P < .05)。kappa值的评判一致性为0.773。尽管2组在术后6小时发生明显疼痛,但2组间的疼痛发生率无显著性差异(P > .05)。术后18小时,MTA组的疼痛评分(6.3 ± 9.5)明显低于CH组(18.5 ± 20.8)。结论:龋源性及可复性牙髓炎牙齿可以通过DPC获得治愈。基于治疗成功率和疼痛程度
