BMC Med :5万人超17年随访研究:不吃肉反而更容易骨折!

先前的流行病学研究表明,素食者的骨矿物质密度(BMD)比非素食者低。这可能是由于钙和蛋白质的摄入与骨骼健康相关联,而素食主义者中钙和蛋白质的摄入量均较低引起的。虽然已有荟萃分析发现钙补充剂可有效地引起BMD的小幅增加,但尚不清楚这种改善程度是否足以降低骨折风险以及素食饮食与骨折风险间的关系。

近日,来自英国牛津大学的研究人员在《BMC Medicine》上发表了题为Vegetarian and vegan diets and risks of total and site-specific fractures: results from the prospective EPIC-Oxford study的研究成果,通过对不同饮食特征组的全部和多个特定部位骨折风险的前瞻性研究,发现非食肉者,尤其是纯素食主义者,发生全骨折或髋部等某些特定部位的骨折的风险更高。

研究人员在英国招募了约54898名参与者,根据饮食特征将其分为肉食组、鱼食组(不吃其他肉)、素食组(不吃肉或鱼,但吃牛奶或鸡蛋)和纯素食主义组(不吃肉,鱼,牛奶和蛋)。同时统计各组参与者的性别、体质指数、烟酒,体育锻炼、饮食补充剂和病史等信息,对其进行定期随访,并通过国家卫生服务记录来统计参与者的健康状况。

参与者的基线特征

在平均长达17.6年的随访期间,共发生了3941例总骨折,566例手臂骨折,889例腕部骨折,945例髋骨骨折,366例腿部骨折,520例踝骨骨折,467例其他主要部位骨折。

使用Cox比例风险回归模型对饮食特征和全骨折及特定部位骨折风险间的相关性进行评估,在调整其他协变量信息后,发现与食肉者相比,素食者的全骨折的风险增加了11%,而纯素食主义者发生全骨折的风险更高,会显着增加50%。

不同饮食组的总骨折和部位特异性骨折的风险

对于特定部位的骨折风险,食肉者与其他三种饮食特征参与者的髋部骨折的风险差异最大。调整BMI后,与食肉者相比,食鱼者、素食者和纯素食主义者的髋部骨折风险均上升,分别显着增加了26%、25%和131%。

其他部位中,纯素食者的腿部骨折和锁骨、肋骨、椎骨等其他主要部位骨折的风险也均高于吃肉者,风险比分别为2.05、1.59、1.02和2.50。不过,四个饮食组间,手臂、腕部或踝部骨折的风险并没有显着差异。

有足够的钙和蛋白质摄入量的参与者中不同饮食组的总骨折和部位特异性骨折的风险

总体而言,与吃肉者相比,不吃肉的参与者发生全面骨折和某些特定部位骨折的风险更高。这些风险差异可能部分是由于非肉食者的平均BMI较低以及钙和蛋白质的平均摄入量较低引起。但是,在调整这些因素后,该风险差异仍然存在,因此可能存在着其他未知的因素,仍需进一步研究明确。
