UOG:双胎减胎术中射频消融是否优于双极电凝

Yinon Y等人进行了一项回顾性队列研究比较射频消融(RFA)和双极电凝(BPC)两种方法在实行选择性减胎术的单绒双羊双胎中的运用,统计观察其围生期不良结局发生率。

该研究共纳入了53例行减胎术的单绒双羊双胎,其中有36例使用了射频消融(RFA)法,17例使用双极电凝(BPC)法。幸存胎儿的整体生存率在RFA组和BPC组相似(88.9% vs 76.5%, p = 0.25)。并且从减胎术到分娩,≤28周、≤34周、≤37周自发性早产率和未足月胎膜早破率在两组间差别无统计学意义。但是BPC组较之RFA组有分娩孕周更短的趋势 (31.3 vs 35.0周, p = 0.09)。因为选择性宫内生长受限而使用RFA进行减胎术的围生期生存率89.5% (17/19),平均分娩孕周为37周。不管是哪个孕周进行的该操作,围生期结局没有差异。对有机儿的减胎,大部分是RFA,围生期存活率100%。

研究结果表明,不管是使用射频消融(RFA)还是双极电凝(BPC)对单绒双羊双胎进行减胎术,其围生期结局无差异。并且RFA更常用于单绒双羊双胞胎并发选择性宫内生长受限时和选择性有机儿减胎时。
